So, this was originally from H&M as Linda has explained a couple of posts down. But there is a VERY similar one currently in Forever 21, and my guess is that it is the same brand as the one from H&M.

Pig Cloth Hoodie
Hayley's hoodie is from Pig Cloth, which is a clothing line run by Aaron Gillespie of Underoath/The Almost and his wife. You can check out their webstore here, but they don't have the black hoodie right now. I assume Hayley got hers from Aaron in person.
